Our practice recognizes the value of integrating mental health care and primary care services to best meet the needs of our patients. Our Psychiatric Nurse Practitioner, Candyce Miller, specializes in the diagnosis and treatment of mental health concerns such as depression, anxiety, bipolar disorder, PTSD, OCD, and ADHD  for ages 18 and up. She is also a Certified Perinatal Mental Health provider with experience caring for women during pregnancy and in the postpartum period. If you would like to make an appointment with her, please complete the Psychiatric History Form below.

Please call the office at 615-859-1440 before your telehealth appointment to "check-in" with our front staff.  YOUR PROVIDER WILL NOT BE ABLE TO CONNECT WITH YOU ON TELEHEALTH UNTIL YOU HAVE COMPLETED THIS STEP. Please give yourself enough time to check-in so you are ready for your provider at your scheduled appointment time. We will need to reschedule your appointment if you do not get checked-in before your appointment time. You may check-in up to 24 hours before your scheduled appointment. ​​​​​
